Being a Home Care Patient Assessment (LPN) coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Obtains initial and ongoing referral documentation from physicians, insurers, and providers. In addition, Home Care Patient Assessment (LPN) initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Requires licensed practical nurse LPN credentials. Typically reports to a manager or head of a unit/department. Home Care Patient Assessment (LPN)'s years of experience requirement may be unspecified.
