POSITION SUMMARY
The Community Resource Coordinator (CRC) is a frontline public health worker who has an understanding of and connection to, the community they serve. The purpose of the CRC position is to provide a link between the patient, health care and social services, as well services offered by the community in which the patient resides. The CRC communicates with all areas of the office to ensure that at risk patients are identified, and assistance provided linking the patient with the necessary resources to ensure the patient receives adequate follow-up care.
Our team-based clinics operate on a Medical Home model. With our progressive primary care team approach, we leverage the expertise from patient care, behavioral health, and pharmacy to provide comprehensive treatment plans. What Youll Do
- Communicate with all areas of the clinic and primary care team to ensure that at risk patients are identified, and assistance is provided
- Facilitate access to services, connections to resources, including follow-up and advocating for patients when necessary
- Identify and recommend solutions to complex patient needs using your expertise and the knowledge and ingenuity of the providers, clinical staff, and your network of social services and community services
- Build cooperative and trustworthy relationships with patients in a professional and compassionate manner. Respectfully deescalate stressful situations when needed
- Participate in professional activities: such as community involvement in healthcare-related activities, professional organizations, committees, continuing education and case presentations

What You Must Have
Education
- High school diploma or equivalent.
- Completion of Community Health Worker training approved by the Oregon Health Authority, preferred
Experience
- Medical office experience, preferred
