Job Description:
We are currently seeking an experienced and detail-oriented Intake Specialist to join our team in Brooklyn, New York. As a vital part of our in-house staff, your primary role will be to oversee the initiation of cases, ensuring all information and documentation are meticulously managed. You will take charge of each case, navigating through our software, and ensuring a smooth transition for patients entering our home care agency.
Responsibilities:
- Case Onboarding: Take charge of the initiation of each case, ensuring accurate and organized management of all information and documentation.
- Software Management: Utilize our software to efficiently navigate and manage cases from admission to discharge.
- Caseload Management: Get the caseload up and running swiftly, demonstrating capability and responsibility in handling multiple cases simultaneously.
Requirements:
- Previous Experience: Minimum one year in a similar Intake Specialist role, preferably within the home care sector.
- Home Care Experience Preferred: Knowledge of home care processes and procedures is highly desirable.
- Responsibility: Take ownership of cases and demonstrate a high level of responsibility in managing tasks and deadlines.
- Self-Starter: Proactive and capable of independently managing caseloads, ensuring all tasks are completed efficiently.
