Work Shift: Day Shift Time Type: Full time Department: CC017060 Health Information Management Summary: Monday - Friday, 8:30 a.m. - 5:00 p.m. — On-Premises Process documents from the patient chart so that they are properly prepared, scanned, and indexed as part of the electronic patient record. Process record requests so that all records are received in a timely manner for patient care, or research. Prepare medical records for scanning. Reviews patient's record for poor quality originals, copies all damaged documents to ensure ease in scanning process. Prepares all documents by removing staples, paper clips, etc., and securely attaches all monitoring strips and other documents to sheets of paper for scanning with no documentation added to the back side of pages. 2. Assembles all documents in appropriate order. Removes all color graphs, photographs, monitoring strips, and replaces them with a photograph cover sheet. Prepares all documents in date sequence from admission to date order correctly except for physician orders, which are in discharge to admission order. Groups like document types together, places in page order. Creates batch for inpatient, outpatient, and emergency room according to procedure. 3. Ensures that the patient name, encounter number, and medical record number are on these sheets. Must understand patient types and admission numbering system to ensure that the appropriate patient medical record/account number is used. 4. Scan the medical record. Places pages through scanner and ensures that each page scans correctly and all pages are positioned correctly and remove any paper jams. Performs routine diagnostics, cleaning and maintenance of scanning equipment. 5. Indexes documents for scanning. Ensure that all scanned documents are positioned correctly and poor quality scans are removed and rescanned. Ensure that each document is indexed to the correct patient/encounter, document name, and verifies that bar coded documents are indexed correctly. Identify appropriate split and merge of documents. Correctly insert or append pages/documents where appropriate. 6. Conducts quality assurance of medical records and complete record requests for continuity of care. Provide requested records to authorized requestors in a timely manner for continuity of care. Perform quality checks on 100% of scanned records, correcting mistakes as appropriate. Log completed record batches into log book, ensuring accurate box numbers. 7. Performs other duties as assigned.
