SUBMISSION REQUIREMENTS
- Bachelor's, Master's Doctorate of Science in Physical Therapy - Required
- Licensed as a Physical Therapist by the Oklahoma State Board of Medical Examiners a Graduate PT eligible for Oklahoma licensure Physical Therapy student in the final semester of graduate school - Required (Must obtain licensure before assuming patient care)
- BLS - Required
