Responsibilities:

The Behavioral Health Coordinator is responsible for acting as a liaison and support person between the Division of Medicaid and Medical Assistance (DMMA) and Case Management team, the Division of Substance Abuse and Mental Health (DSAMH) and Behavioral Health providers. The Behavioral Health Coordinator will be responsible for outreach activities with participants and providers and will be responsible for documenting all activity in electronic case management documentation system. 

The Behavioral Health Coordinator is responsible for:

• Coordinating Participant care needs with BH Providers.
• Developing processes to coordinate behavioral healthcare between Primary Care Physicians and Behavioral Health Providers.
• Participating in the identification of best practices for BH in a primary care setting. 
• Coordinating behavioral care with medically necessary services. 
• Being knowledgeable of the PROMISE requirements and coordinating with other BH-MCOs to effectuate the requirements.  
• Carry a small caseload of complex behavioral health needs. 

Education/ Experience:

• Master’s Degree Required 
• Active Delaware Licensed Clinical Social Worker (LCSW) required
• Valid driver’s license
• 3 to 5 years prior experience in behavioral health/human services required; or other equivalent background and experience that would translate well to this position

Other Skills:

• Customer Service and previous telephone experience preferred
• Proficiency with Microsoft Office Suite (Word, Excel, Power Point); MS Access is a plus
• Ability to travel up to 25% throughout state of DE
