Job Title: Lead Prevention Navigator
Job Summary:
Lead Prevention Navigator will act as task supervisor for Prevention Navigator team and will provide oversight and guidance while in the field. Lead PN will work with Director and PNS team to spearhead outreach efforts to increase prevention service availability to those who are HIV or at risk for HIV and S/U/COD. Lead Prevention Navigator will ensure that the team is conducting 12 GPRAs a month, following up with all GPRAed clients at 90days and proving 50 HIV/HCV test per month. Lead PN will ensure that he/she and Prevention Navigation provide services for HIV and at risk population via Prevention Navigator, Health Educator and Peer and will oversee referrals for food stability, medical and medication adherence, STI referrals, SU/ COD prevention and referral to CATCH Treatment team if/when appropriate.
Key Functions/ Responsibilities
Primary Duties & Responsibilities include, but are not limited to:
· Lead PN reports to Director of CATCH, EIS NYC and PNS.
· Lead PN will received basic patient navigation/community health worker training from a recognized program such as the New York-Presbyterian Hospital Center for Patient Navigation. Trained in Motivational Interviewing, Stages of Change, trauma-informed services, and Theory of Harm Reduction. NYS Mandated Reporter for Child Abuse and Neglect.
· Lead PN will coordinate referrals and access to care for HIV and high-risk for HIV clients who are in crisis due to substance misuse, co-occurring mental disorders, and unstable housing with access to comprehensive services for indicated medical care, behavioral health care, stable housing, and other support services while maintaining compliance with confidentiality regulations including NYS OASAS 42 CFR. Support client education on safer Behaviors, Engagement and retention in confirmatory testing and treatment for HCV, HAV, HBV.
· Lead PN will ensure that 12 GPRAs per month are completed and 50 rapid HIV/HCV test per month administered.
· Use culturally competent practices to work with care management teams of programs with which CATCH/EIS has linkage so that crises are resolved, clients are stable participants in treatment plans and are on the path to treatment adherence and recovery. Provide follow-up at 90 days to document client sustained engagement in care.
· Maintain timely AWARDS database records that delineate involvement in the service and are maintained in accordance with applicable state and federal confidentiality law.
· Attend all required staff meetings and trainings.
· Must be able to perform fieldwork in the Bronx, Manhattan, Brooklyn, and Queens.
· Must be available to work 35 hours per week Monday to Friday.

Qualifications/Criteria:
· Bilingual English/Spanish preferred, at least 3-years’ experience with persons affected by HIV SUD/COD, and/or homelessness with some leadership experience

Education:
·Associates degree or 3 additional years’ experience.
Location: work in all five Boroughs based out of 760 east 160th street
