Based in our Salisbury South, as our Home Care Package Admissions Coordinator, you will be responsible for providing easy to understand information and explanations for the benefits of a Home Care package to clients and their families.

The HCP Admissions Coordinator will be supporting older individuals with complex aging concerns to maintain their independence and continue living in their own homes.

You’ll also make an impact by :

• Developing individualised Agreed Support plans based on the assessment results, taking into account client preferences and goals.
• Establishing and maintaining effective community networks with key stakeholders.
• Creating opportunities to ensure Helping Hand maintains a positive profile and image in the community and industry.
• Ensuring accurate, relevant, and timely documentation that aligns to Home Care’s use of defensible documentation and the FACT (facts, actions, communication, times) method of documenting.

What you’ll bring

To succeed in this role, you will have :

• Diploma in Community Services or equivalent work experience
• A strong theoretical approach to working with ageing population and demonstrated practical application of this approach
• Own car or access to vehicle and current driver’s licence
• Valid NDIS Worker Screening clearance or be willing to obtain one
