The Discharge LVN will be responsible for providing discharge education and instructions to patients, families, and caregivers to improve patient flow in the acute care medical-surgical setting. The Discharge LVN will work collaboratively with the Throughput Supervisor and other healthcare professionals and is accountable for ensuring patients are discharged in an effective and efficient manner including resolving any complex needs. Responsible for the improvement of the discharge process by expediting order to door times, reducing barriers to discharge by communicating with the Throughput Supervisor and the Discharge Lounge Case Management Assistants.

QUALIFICATIONS
License /Certification
Required:
CA Licensed Vocational Nurse
BLS
Experience
Preferred: Minimum of one year of clinical experience in an acute care setting, with experience in discharge planning and patient education preferred.
Knowledge/Skills/Abilities
Strong clinical assessment and critical thinking skills, with the ability to recognize and address potential barriers to successful discharge.
Excellent verbal and written communication skills, with the ability to educate and empower patients, families, and caregivers.
Knowledge of community resources and referral processes.
Ability to work collaboratively with interdisciplinary team members and healthcare professionals.
Strong computer skills, with proficiency in electronic medical record documentation.
JOB RESPONSIBILITIES
Essential
Provides individualized education and instructions to patients, families, and caregivers on medications, diet, activity level, wound care, and follow-up appointments.
Collaborates with interdisciplinary team members to implement discharge plans that meet the individual needs of each patient, including referrals to community resources.
Utilizes clinical knowledge and judgment to recognize and address potential barriers to successful discharge and follow-up, such as transportation, financial, and social needs.
Provides accurate and timely documentation of discharge education and instructions in the patient's medical record.
Participates in quality improvement initiatives related to discharge processes and patient outcomes, including data collection, analysis, and feedback and via collaboration with the Throughput Supervisor.
Communicates effectively with patients, families, and caregivers to ensure understanding and compliance with discharge instructions.
Maintains current knowledge of evidence-based practice related to discharge education and instructions, and participates in continuing education and professional development opportunities.
Serves as a resource to other healthcare professionals in the delivery of effective discharge education and instructions.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
Performs other duties as assigned.

$27.35 - $41.03
