Provides comprehensive case management for individuals at risk of or living with HIV. May include advocacy, risk reduction education and counseling, and providing emotional support to individuals engaging in or at risk of engaging in sexual and substance use behaviors, including sex work, intravenous drug use and other behaviors that increase risk of harm.

Facilitate regular support groups for consumers. Will include routine HIV testing and Mobile Health Center (MHC) collaboration.

Will involve field work including home visits and accompaniments as needed. Expectation For this position is that time will be spent 60% of time completing case management requirements and 40% focused on MHC requirements.

  • This is a full-time position located in New York City at Project Street Beat (offices in Brooklyn and the Bronx), within PPGNY Region One.
  • o Region One - NYC Nassau County New York, Nassau, Oneida, Kings, Queens, Richmond, and Bronx DUTIES AND RESPONSIBILITIES (WEIGHTED & WITH DETAIL) Program and Office Support (55%) ·Determines individual’s eligibility for case management and other PSB services;
  • Responsible for conducting and documenting PSB intake, risk assessments and reassessments, service plans and ongoing services;
  • Provides emergency concrete services including but not limited to food, transportation and accompaniments to necessary services;
  • Intervenes with clients by educating them about the risk of HIV / AIDS, the need to protect themselves through the use of various risk reduction strategies, and provides them tools and information on proper usage;
  • Provides ongoing case management including; risk reduction counseling regarding sexual and substance use, linkages to medical, social and supportive services, referrals for housing and related services, follow-up / reengagement, accompaniments and home visits;
  • Facilitates support groups and group-level interventions. ·Participate in regular outreach and recruitment events on the Mobile Medical Health Center (MHC) at a minimum of twice a week.
  • Participate in engagement and education of priority populations regarding HIV Counseling and Testing, PrEP, and access to health care and other supportive services such as Opioid Overshoe Prevention training and the distribution of ESAP (Expanded Syringe Access Program) Administrative Team Coordination (10%) ·Accepts individual and joint responsibility for and participates in departmental efforts to comply with all organizational and quality assurance guidelines, and all applicable government regulations;
  • Enter complete required data into required data systems within 48 hours of completing a service, current PSB data systems include but are not limited to EPIC Efforts to Outcomes (ETO) and eShare.
  • Works effectively with volunteers, PSB Health Promoters, trainees and temp agency personnel. ·Work collaboratively with the HIV Prevention Specialists, Case Managers, Patient Navigators and Health Promoters to engage, enroll and facilitate services : HIV Counseling and Testing, PrEP, Seeking Safety, Overdose Prevention training and harm reduction services·Meet monthly service delivery goals·Maintain all PSB equipment.
  • Including but not limited to cell home ad laptopBudgetary / Statistical Support (10%) ·Accountable to meet all policies and procedures for service delivery and documentation;
  • Accepts individual responsibility for program spending (, program supplies, meals, etc.), distribution of incentives and documentation, according to the spending plan and PSB Incentive Distribution Policy .

Customer Service Responsibilities (15%) ·Arranges for the provision of health services for clients either through the program's Mobile Health Center, other PPGNY health centers, or through referral to other providers.

Human Resources (5%) ·Completes all responsibilities according to established protocols, policies and standard practices in the areas of ADA and regulatory compliance programs such as HIPAA.

Special Projects (5%) ·Performs other educational and experience related duties as required.

REQUIRED SKILLS / ABILITIES :

  • Excellent organizational skills and attention to detail;
  • Proficient in Microsoft Office Suite or similar software;
  • Excellent written and oral communication skills;
  • Must be comfortable utilizing public transportation.

QUALIFICATIONS

  • Minimum Bachelor’s degree or CASAC. Minimum 1 year in case management services.
  • Sensitivity to cross cultural issues
  • Familiarity with social services.
  • Familiarity and comfort with harm reduction model.
  • Strong interpersonal skills.
  • Preferred experience working with issues such as homelessness, criminal justice system involvement, HIV / AIDS, substance use and mental health issues;
  • Preferred knowledge of health care delivery system and public health benefit programs;
  • Familiarity with PSB centered communities
  • Able to maintain confidentiality and HIPAA compliance.
  • Fluent in Spanish and / or other languages preferred.
