Responsibilities
As a Medical Records Clerk responsible for EMR conversion, you will:
- Process patient paper medical records and efficiently convert them into Electronic Medical Records (EMR) for access by physicians, clinicians, and other healthcare staff.
- Scan documents and file them into the Health Information Management (HIM) system with meticulous attention to detail.
- Verify that all pages have the appropriate patient identification and form identification barcode.
- Conduct thorough reviews of paper records against electronic documents to ensure image quality and identify any missing documents.
- Validate and rectify image/document errors as needed to maintain data accuracy.
- Accurately identify the destination of records and/or loose reports.
- Adhere to defined schedules and expectations as set by HIM Management.
- Maintain productivity and quality levels in accordance with current standards.
- Retrieve and scan discharged records from various units and outpatient departments, preparing them for transport.
- Utilize designated discharge logs to verify receipt of records on respective units.
- A High School Diploma.
- Previous experience in scanning and clerical work.
- Knowledge of medical terminology.
- Computer skills, including proficiency in Microsoft Office Word, Outlook, and Excel.
