The Palliative Care Social Worker should be able to perform the following tasks:
- Use a biopsychosocial assessment of the patient and family as a basis for care planning
- Facilitate or co-facilitate family meetings
- Facilitate effective communication between patients/families and the IDT
- Advocate for effective team dynamics, including conflict management, wellness, and self-care
- Assist patients with advanced care directives
- Debrief difficult cases or deaths within the team
- Educate patients/families about coping with illness, normalizing stress, and palliative care options
- Identify and report abuse and neglect as mandated by law
- Identify resources, provide practical assistance, aid with insurance coverage, navigate systems, and plan funerals
- Document assessments, progress, and response to treatment as required by organizational and professional standards
- Provide financial counseling related to the cost of care
- Improve care delivery for persons with life-threatening illness
- Provide individual and family counseling specific to cancer, coping, and planning
- Educate IDTs about mental health issues, family dynamics, or psychosocial factors impacting care
- Assess and address caregiving resources and needs
- Advise palliative care members on maintaining appropriate and therapeutic boundaries
- Intervene in crisis situations, including suicide assessment and prevention
- Facilitate and participate in research and quality improvement activities
- Collaborate on discharge planning
- Cultivate community relationships and develop community resources
- Conduct legacy work, including dignity therapy and Acceptance
- Influence and improve social policy and ethical practice related to palliative care
- Provide bereavement care and grief counseling, including anticipatory responses to and processing of grief
- Have awareness of state laws and regulations regarding end-of-life care.
