JOB SUMMARY:
Transitions at Home Registered Nurse Case Managers provide skilled nursing care, including performing assessments, administering medications and treatments, coordinating services, and counseling the patient and family/significant others in their place of residence. This is performed according to physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Full-Time 30-40 hours per week M-F; on-call rotation
REQUIREMENTS:
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance

JOB DUTIES:
- Travels to patient homes and provides skilled nursing care within the established plan of care, such as injections, IVs, wound care, lab draws, PICC care, ostomy care, completing vital signs, medication managements, and performing physical assessments of the patient's overall medical condition.
- Continually evaluates the patient's condition and the effectiveness of treatments, plan of care, medications, and therapies.
- Conducts patient admissions and discharges.
- Works in collaboration with the RN Clinical Manager and other patient care team members to ensure coordination of care is provided to attain desired outcomes.
- Documents visit notes, treatments, and care. Records and reports symptoms and changes in patient's condition. Verifies that physician orders are complete, accurate, and cover the care and visits provided.
- Provides information, instructions, and education to the patient, patient family members, and caregivers.
