What are the responsibilities and job description for the Intake Coordinator - 88289 position at UHS?
The Intake Services Coordinator, under clinical supervision of a licensed social worker or licensed professional counselor, coordinates the referral development and pre-admission patient assessment process.
- Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW
- Facilitates groups, family and individual sessions and completing all necessary documentation
- Schedules Unit assessments, meetings and appointments as appropriate
- Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW
- Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
- Reviews newly admitted patient accounts for eligibility and authorization daily
- Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
- Participates as an active member of the interdisciplinary team
- Addresses issues in a timely manner including crisis and higher level of care referrals.
- Maintains appropriate ethical and clinical boundaries with client, family members and other staff members.
- Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon’s confidentiality policies and procedures.
- Maintains a professional code of conduct as established by Horizon’s Code of Conduct.
- Appropriately represents the Program at public and private outreach events
- Attends Unit and Hospital meetings as directed
- Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested
- Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data.
- Utilizes data to complete program’s key statistical reports and generates reports as needed for the SVPO, VPCE, program director and client hospital leadership when requested
- Promotes and develops positive, professional interaction with patients and their families, peers, leadership, clients, etc.
- Corrects issues identified and reports errors to the Program Director
- Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission
- Prepares and disseminates monthly reports and completes required follow up on activities on behalf of the Director. Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices
- Regularly collects data to provide input and/or feedback to appropriate customers/suppliers (i.e., teams, supervisors, co-workers, vendors, nursing agencies) to improve work
- Other duties as assigned

Qualifications
- A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or other related field is preferre
- Minimum three years’ of experience in a similar position and/or industry
- Proficient in software applications
- Excellent written and verbal communication and presentation skills
- Ability to read and comprehend moderate to complex instructions and correspondence
- Ability to write detailed correspondence
- Ability to effectively present information in one-on-one and small group situations to patients and their families, Program leadership, employees, clients, providers, vendors, etc.
