SHIFT: No WeekendsSCHEDULE: Full-time
Wesley Woodlawn Hospital & ER - Wichita, Kansas
Build relationships with patients and physicians, to coordinate a plan of care including appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary care environment
On an aggregate level, tracks performance of the program in line with HCA, division and facility goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practice, through development of evidence-based guidelines
Assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each patient including support for interdisciplinary and cross facility collaboration (i.e. fetal boards, and communication with the referring physician
Serves as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, support network members e.g. social workers, and the wider healthcare community; Conducts internal and referral for external outreach and marketing; May include health fairs, screenings and other special events
Assesses the medical, social, and psychosocial and other care needs of the patient and family
Assess and initiates referrals to the hospital and community resources to connect patient as needed
Provides appropriate teaching, outreach, and education to patients and families to ensure the patient is empowered to manage his or her own health
Establishes algorithms, documents, and formalized processes for transition in commonly followed care pathways
Conducts follow-up conversations as needed with all patients to communicate concerns, changes, or social needs in patient health to appropriate MD or other care providers
Supports and conducts outreach to referrers, providers, other medical professionals and the community
Organizes and attend patient education classes and other meetings as necessary; including but not limited to the Bariatric Surgery Support Group
Tracks and documents interventions and outcomes, including managing and updating the Bariatric dashboard
Create and conduct a comprehensive nutrition assessment to adapt nutrition care plan to a patient's clinical, financial, social, and health literacy status while providing care.
To optimize patient outcomes, this individual will screen for pre-existing nutritional deficiencies, develop dietary interventions, and prepare patient for post-operative diet changes.

Required Registration, Licensure or Certification
- Current Licensure in the State of Kansas as a Registered Dietitian (RD)
- Current Basic Life Support (BLS) verification course, as specified in policy
Preferred Registration, Licensure or Certification
- Successfully completes the MBSCR Online Training Program and subsequent Certification
- Experience in program development
- Member of the Academy of Nutrition and Dietetics
- Weight Management Counseling
Required Education or Experience
- Bachelor's Degree
- Minimum 2 years of experience in nutrition services.
Required Knowledge and Skills
- Collect and Submit accurate, complete and timely data to the MBSAQIP data registry platform.
- Develops nutrition care plans, including specifications, methods and timetables for the Bariatric Program depending on patient and resident status and needs.
- Directs nutritional care of patients and residents with special needs including screening, assessment, intervention, and plan development to ensure the patients nutrition needs and customer satisfaction levels are met.
- Stays current with most nutrition-related trends, research, policy, certifications, and standards of professional practices.
- Ensures compliance with all federal, state and local regulations as well as Wesley's policies and procedures (HIPPA/Privacy Act, quality assurance, safety, operations, personnel).
- Participates in program teleconferences.
- Participates in data compilation for audits and (re)accreditation or site visits.
- Participates in program teleconferences
- Works effectively with Hospital and Individual surgeon Office Staff
- Periodically attend voluntary in-person professional development seminars offered at various annual meetings such as ASMBS, ACS NSQIP, or ACS Clinical Congress.
