The Therapy Case Manager, manages all aspects of accepting and managing all therapy referrals, the field therapists schedule based on a patients plan of care, daily patient referrals as well as manage the schedule for existing patient schedules. This is a temporary position with potential to extend into a permanent one. DUTIES AND RESPONSIBILITIES: Schedules field therapists appropriately to patients in accordance with the patients plan of care. Assists with new employee orientation Coordinates all therapy for the patient including scheduling patient visits and coordinating services. Reviews and understands the admission process for the plan of care of each patient. Coordinates plan of care with therapy providers. Reviews therapy evaluations and visit notes. Follows Medicare regulations. Maintains a good working relationship with all contracted companies. Assists with additional scheduling responsibilities as needed.
