Responsibilities Wekiva Springs Center in Jacksonville Florida has an immediate opening for a PRN Case Manager to work 8-16 hours per week. The Case Manager is a member of the multidisciplinary treatment team. Major functions include conducting psychosocial and CD assessments, gathering info from family and other providers, identifying discharge needs, and complete discharge planning.
