JOB SUMMARY: The Resident Nursing Assistant (RA) is responsible for providing direct care to residents, assisting the clinical team in providing activities of daily living for the residents and works under the supervision of a licensed nurse. This person works in accordance with facility policies and procedures and report resident needs and concerns to a licensed nurse. RESPONSIBILITIES AND MAJOR DUTIES: 1. Timely and accurately document all resident related activities in accordance with facility policy. 2. Ensure timely notification of resident related information, including changes in resident condition (example: weight loss, skin condition changes), to a licensed nurse for further direction. 3. Successfully assist all responsible residents with Activities of Daily Living (ADL's), including but not limited to feeding, bathing, dressing, transferring, ambulation, loco motion, personal hygiene, toileting. 4.
