Transitions Rehabilitation was established in 1999 in Southern Oklahoma as Red River Rehabilitation. Initially we served four skilled nursing facilities and one outpatient facility. Red River Rehabilitation joined Senior Care Group in 2006. In 2008 our name was changed to Transitions Rehabilitation and we expanded to serve the facilities owned and operated by Senior Care Group in Tulsa, Oklahoma, North Carolina and Florida. Today, we provide therapy services in 13 skilled nursing facilities and two outpatient clinics, as well as providing therapy services in the local school systems in Southern Oklahoma. We also work closely with Diamond Home Health to provide home health therapy services in the Tampa Bay community. Transitions Rehabilitation employs over 165 full time, part time and pool therapists and provides therapy services to over 300 patients a day. We accept Medicare, Medicaid and many insurance plans!
