Transitions Rehabilitation was established in 1999 in Southern Oklahoma as Red River Rehabilitation. Initially we served four skilled nursing facilities and one outpatient facility. Red River Rehabilitation joined Senior Care Group in 2006. In 2008 our name was changed to Transitions Rehabilitation and we expanded to serve the facilities owned and operated by Senior Care Group in Tulsa, Oklahoma,
