You will be responsible for facilitating groups, counseling clients on services available for treatment, helping combat relapses, and building long-term life skills to maintain sobriety. Responsibilities: -Conduct groups and 1:1 counseling sessions with clients per program standards and document progress and participation. -Obtain clinical information from the intake assessment, medical hx questionnaire, biopsycho-social assessment and use the information to develop and implement a collaborative treatment plan. -Maintains thorough clinical records for clients, including all services provided as part of individualized episode. -Prepare progress reports and letters for relevant parties, if applicable. -Educate families and groups on substance abuse treatments and prevention programs. -Facilitate referrals to other healthcare or counseling professionals. -Assists the client in discharge planning
