Interim Service Coordinator

  • Position Summary: Ensure the implementation of the WV Birth to Three Individualized Family Service Plan process, including: explaining the process and options available, facilitating meetings, keeping record of services offered, and linking families to relevant services.

    Essential Functions*

    • Manage an assigned caseload by conducting intake services such as scheduling and conducting intake services in families’ natural environments, conducting referral follow ups, and upkeep of required paperwork and documentation.
    • Facilitate meetings with families and stakeholders by planning, scheduling, leading discussions, and completing documentation to develop Individualized Family Service Plans and Transition Plans.
    • Establish and maintain mutually respectful partnerships with agencies, practitioners, and community partners
    • Remain in a stationary position up to 50% of the time.
    • Traverse various terrain to access visitation sites.

    Marginal Functions

    • Assist in recruiting practitioners in areas of need.
    • Participate in ongoing monitoring and continuous improvement activities related to the performance of RAU responsibilities.
    • Actively participate in child find and public awareness activities.
    • Any additional duties as assigned.

    Skills, Abilities, & Knowledge

    • Abide by all applicable Federal, State, and local laws, rules, regulations, and policies related to the program and relevant grants.
    • Ability to use Microsoft Office products and conferencing technology to conduct and participate in meetings
    • Proficient in early childhood development
    • Strong case management skills, intake and assessment skills
    • Flexibility: willing to see families in a variety of settings (community, home, evenings), work a flexible schedule as needed, and work independently.
    • Excellent verbal and written communication skills.
    • Excellent interpersonal, negotiation, problem solving, and conflict resolution skills.
    • Excellent organizational skills and attention to detail.
    • Excellent time management skills with a proven ability to meet deadlines.
    • Ability to act with integrity, professionalism, and confidentiality.

    Education

    • Required: Bachelor’s degree in an approved human services field such as social work, elementary education, special education, nursing, psychology, counseling, sociology, speech and language pathology or approved equivalent program of study OR Bachelor’s degree in a non-related area with at least 9 hours of related coursework in an approved human services field such as social work, elementary education, special education, nursing, psychology, counseling, sociology, speech and language pathology or equivalent program of study.

    Experience

    • Preferred: One-year relevant occupational experience working directly with children ages 0-3 and families from multi-cultural and diverse socio-economic backgrounds.

    Employment Conditions

    • Valid driver’s license.
    • Able to travel extensively and work flexible hours.
    • Adhere to the National Association for the Education of Young Children (NAEYC) Code of Ethics.
    • Upon hire, register for the WV STARS career pathway and complete ongoing trainings to maintain enrollment.
    • Maintain Interim Service Coordinator credential.

    Environmental Conditions

    • Indoors in a normal office environment with little exposure to excessive noise, dust, fumes, vibrations, and temperature changes at least fifty percent (25%) of the time and in a non-traditional setting 75% of the time.
    • Prolonged sitting at a desk viewing a computer screen and keyboard typing.
    • Frequent face-to-face, electronic, and virtual interactions with internal and external customers.
    • Frequently work at a fast pace with unscheduled interruptions.
    • Public contact position requiring appropriate business casual apparel.
    • This position may be eligible to work remotely up to two days per week following a successful 90-day review.

    Physical Demands

    • Mobility within the work setting including movement from floor to floor.
    • Access information using a computer.
    • Must be able to lift 25 pounds at times.
