    Full Job Description
    (Children-Adolescent-Adult Clients)
    The position responsibilities include providing comprehensive targeted case
    management services to Mental Health clients including:

    • Provide case management functions of assessing, planning, linking, monitoring and advocating for individuals including coordinating and monitoring the activities of the individual.
    • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall service plan goals as well as ensure immediate changes are made in service plans as individual’s’ needs change; educate and support clients’ families; and advocate for clients’ rights and preferences.
    • Provide outreach services to the individual in their natural environment to foster and support the case management relationship and to ensure engagement in their service planning.
    • Document all activity in progress notes and complete all other documentation as required by 10E-15 Florida Administrative Code and Medicaid community mental health and targeted case management service guidelines to maintain a permanent record of client activity according to established methods and procedures.
    • Help clients access adequate financial support, reliable transportation or provide transportation or assistance with travel arrangement to foster greater mobility within the community, vocational rehabilitation, primary health care, maintain a safe and affordable place to live, and various other community resources aimed at maximizing the independence and productivity of the individual.

    • Required Qualifications

    Experience Mental Health Targeted Case Manager that meets the following
    criteria will be considered for interview only:
    1) Must hold a CBHCM Certification from the Florida Board or have a provisional
    certification in process of completing requirements for complete certification.
    2) Must have previous Experience at Mental Health Targeted Case Manager
    (CBHCM) for a minimum 1 year of proof experience. .
    3) Must hold a Bachelors degree in medical field, behavioral science, human
    services or related field.
    4) Communicate in and comprehend the English language effectively both orally
    and in writing.
    5) Bilingual: Spanish-Creole-French, Must be proof fluent in languages and a
    great grammar skills.
    6) Work flexible hours as required by program and staffing needs and display
    willingness to be flexible and work cooperatively with co-workers.
    7) Valid Florida driver’s license with a clean record. Be able to provide proof of
    having automobile liability coverage.
