The Case Manager provides case management services to children, adolescents and their families.

All Agency staff are expected to focus on Quality improvement as a part of their job responsibilities, in an effort to make the Agency a safe and healing environment for clients and staff. This expectation includes, but is not limited to: teamwork, improving service delivery, fulfilling department goals, and other job-specific tasks that encourage excellence.

ESSENTIAL DUTIES and RESPONSIBILITIES: It is the responsibility of each employee to be a consistent witness for Jesus Christ and to adhere to the Standard of Moral Conduct and Statement of Faith. Further, each employee is responsible for dealing with others with a Christ-like attitude while helping them experience their worth in Christ, and for demonstrating behaviors reflecting CCHO's core values of Relentless Commitment, Selflessness, and Kindness.

Other essential functions include the following:

  • Makes every effort to maintain a 'team first' spirit and atmosphere within the workplace environment fostering and maintaining appropriate working relationships within the Encompass team, and with other Agency departments, as well.
  • Case Management services are focused on the client's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family as well as integration and contribution within the community.
  • Duties of the Case Manager may be conducted in person or by phone and will include those designated by the above Ohio Code:
    • Ongoing assessment of needs
    • Assistance in achieving personal independence in managing basic needs
    • Facilitation of development of daily living skills
    • Symptom monitoring
    • Coordination and assistance in crisis management and stabilization
    • Advocacy and outreach
    • Education and training specific to the needs of the client and family
    • Interventions that eliminate barriers to seeking or maintaining education and employment
    • Activities that increase the client's capacity to positively impact his/her environment
  • Completes or assists with treatment plans (ITPs) in collaboration with client, formulating goals, objectives, and interventions relevant to client's diagnosis; and reviewing at least quarterly ongoing medical necessity of those goals (and/or diagnoses) in conjunction with client's progress in treatment.
  • Aids clients in making and carrying out objectives in accordance with their mental health treatment plan, as appropriate for Case Manager role.
  • Participates in Quality Improvement activities, including, but not limited to, peer review and case record reviews.
  • Provides input and feedback, based on observation of clients, to contribute to therapist's development of mental health assessments and ITPs.
  • Completes timely and well-written documentation in the CareLogic EHR for all clients.
  • Attend wrap-around meetings and family team meetings, as needed
  • Attend agency meetings as needed, including, but not limited to staff meetings, clinical and/or small group supervision, as required, to increase & maintain competencies in mental health field.
  • Engage in on-going training and maintenance of licensure and/or mental health credentials.
  • Provide on-call services, as needed.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily. EDUCATION and/or EXPERIENCE: High School diploma required. Bachelor's degree preferred, with strong preference given to a Bachelor's degree in Social Work or Psychology. Three years' experience in mental health or related field strongly preferred (required without bachelor's degree); experience working with children and families strongly preferred.

LANGUAGE SKILLS: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

COMPUTER SKILLS: To perform this job successfully, an individual should have ability to use a computer, with special familiarity with Microsoft Office suite, email and navigating the internet.

CERTIFICATES, LICENSES, REGISTRATIONS: Valid Ohio Driver's Licenses and maintains a driving record that allows that individual to be insurable with the insurance company

OTHER SKILLS and ABILITIES: Ability to use fax machine, copier, printer.

The employee must occasionally lift and/or move up to 50 pounds. The ability to cope with and tolerate moderate levels of stress is also a necessity. Specific vision abilities required by this job include close vision, distance vision, depth perception and ability to adjust focus. This job requires the employee to maintain licensure and drive a motor vehicle in the state of Ohio.
