OVERVIEW:

We are looking for a Full Time Hospice Care Coordinator to join our team.

 

Under direction of the Director of Business Development, the Hospice Care Coordinator serves as the liaison between Hinds Hospice and healthcare professionals in hospitals, long-term care facilities, assisted living communities and physician offices within Fresno, Madera and Merced counties.  While doing so, identifies key personnel in each office, builds and maintains relationships by providing Hinds Hospice education and appropriate educational materials for referrals of hospice appropriate patients. The success of this position derives from the provider relationships that result in referrals of hospice eligible patients.

 

  • Determine potential referral sources, then contact and educate about Hinds Hospice services.
  • Coordinate and conduct presentations on end-of-life care to healthcare professionals and the general community. Coordinate general community education and outreach activities (e.g. health fairs, United Way campaign, etc.) to promote Hinds Hospice programs and services.
  • Promote public relations in all activities of the organization (e.g. fundraising events, on-going promotions, thrift stores, new programs).
  • Develop and foster relationships with existing referring and non-referring physicians and other healthcare providers providing them information on Hinds Hospice end-of-life care services.
  • Demonstrate proficiency in knowledge of the holistic philosophy of end-of-life care, and Hinds Hospice programs and services.
  • Create marketing plans for assigned territory and is accountable for the goals of that plan.
  • Expected to contribute to the development and improvement of marketing and educational materials for presentations.
  • Determine and meet with potential referral sources who influence or make referral decisions, then contact and educate about Hinds Hospice services.  This will include preparing regularly scheduled visit rotations and outreach goals on how to increase referrals and maintain facility contacts.
  • Maintain daily documentation in Customer Relationship Management (CRM) program, including daily and weekly call schedules. Record all vital information including names, topics, follow-up needed and expenses incurred.
  • Perform non-clinical education with patients and their families regarding hospice services and completion of consent forms.
  • Coordinate and conduct presentations on end-of-life care to healthcare professionals and the general community.  Coordinate general community education and outreach activities (e.g. health fairs, United Way campaign, etc.) to promote Hinds Hospice programs and services.​​​​​​​
  • Promote public relations in all activities of the organization (e.g. fundraising events, ongoing promotions, thrift stores, new programs).​​​Develop and foster relationships with existing referring and non-referring physicians and other healthcare providers providing them information on Hinds Hospice end-of-life care services.​​​​​​​
  • Maintain updated database for all contracts made in the medical community.​​​​​​​
  • Provide training and education for Case Managers, Discharge Planners, Social Workers and other key personnel within healthcare facilities on Hinds Hospice end-of-life care.​​​​​​​
  • Provide in-service education to contracted facilities with a hospice R.N. educator, as needed, to promote education on hospice care to foster positive working relationships and increase referrals from the facilities.​​​​​​​
  • Demonstrate proficiency in small and large group presentations.​​​​​​​
  • Research current trends in end-of-life care, and update educational materials used.​​​​​​
  • Meet regularly with Director to discuss recent activities, propose goals, develop plans to increase outreach and referrals, and for approval of marketing materials for assigned territories.  Make revisions to weekly plans accordingly.​​​​​​​
  • Be prepared to experience medium to heavy lifting, extreme outdoor weather conditions, and considerable time traveling in a motor vehicle, along with some evening and weekend work, all of which are necessary to perform the essential functions of this position.​​​​​​
  • Serve as a persuasive and proactive advocate for patients and caregivers who are in need of hospice care and grief and loss services.​​​​​​

YOU HAVE


 

  • Bachelor’s Degree with a minimum of one year experience in Hospice Care Coordination is required.
  • Experience in marketing/sales highly desirable.
  • Excellent communication skills.
  • Ability to interact with the public, vendors and other employees in a professional manner required.
  • Must have a valid CA driver’s license and a willingness to use personal vehicle in the course of employment.
  • Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period).
