Being a Case Manager (LPN) collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Additionally, Case Manager (LPN) develops a discharge plan that provides the best available resources to meet ongoing patient needs. Identifies patient care issues and makes recommendations. Requires a high school diploma or equivalent. Requires graduation from approved LPN educational program. May require a state license. Typically reports to a Registered Nurse (RN) or supervisor.
