The Center for Respite Care offers a Housing and Urban Development funded, scattered site, housing program for individuals who are chronically homeless and have one or more disabilities. It is a "housing first model," and allows the individual to remain housed permanently while receiving intensive case management and assistance with the coordination and utilization of community supports. Key interventions of the Permanent Supportive Housing program include: Meeting immediate needs of homeless - The Center provides access to, and funding for a market rate apartment in the Greater Cincinnati area. Transition to Self Sufficiency - The program also focuses on becoming an important link in the transition to self-sufficiency. The Center provides an array of services to assist individuals in developing and implementing a case plan including application for entitlement programs, coordination of plans with external case management, and referrals to treatment programs, income, substance abuse and mental health services.
