Roles & Responsibilities: • Perform consultations based on the results of biometric screenings, such as blood pressure, BMI, height and weight measurements, cholesterol and glucose screenings, and osteoporosis screenings. • Conduct Mild Cognitive Impairment screening, PHQ-2 screening, medication adherence discussion, and smoking cessation during the consultation. • Provide a list of local providers available on the iPad.
