Evidence suggests that COVID-19-related stress and isolation has had a pernicious effects on those with behavioral health conditions, particularly opioid use disorder. Government and behavioral health providers are, in essence, waging war on two fronts necessitating a response that is as comprehensive as it is cooperative.
Access: Supports for Living Inc. will be providing a OUD team, embedded at the County 911 Center and tangential to the County’s Mobile Mental Health Team. This team will enhance the coordinated effort to combat opioid use disorder (OUD) in Ulster County. The enhanced team consists of a Registered Nurse Care Manager, Clinical Team Leader, and psychiatrist.
Job Description:
Access: Supports For Living is seeking a Full-Time RN to help lead this new project. The RN Care Manager is the most integral component of the team. Working in partnership with any and all other opioid response efforts, including Oracle, County Opioid Response Teams, Emergency Departments, EMT/Ambulance and 911 Center, the RN Care Manager will coordinate care for people who identify as having an OUD.
Duties include but are not limited to:
· Meet face-to-face with the person and their circle of support at home or in the community
· Provide the necessary assessments and linkage to psychiatry
· Collect samples and coordinate with laboratories as needed for MAT
· Provide nursing support, per doctors’ orders
· Assist with insurance coverage issues
· Assist with medical records all with aim of removing logistical barriers to treatment.
The RN Care Manager will work in concert with the psychiatrist, who will supervise medical care. When direct services are provided by the psychiatrist, visits will be billed under Access’s Article 31 clinic. When necessary, psychiatric services can be provided temporarily for uncompensated care until insurance is secured.
Equally as important, the RN Care Manager will facilitate communication between providers to avoid duplication of efforts and promote continuity of care; this includes referrals to the County’s Mobile Mental Health Team and/or other community resources for either the person presenting with an OUD or their family.
Qualifications:
-Current, valid NY RN license
-Associates degree Bachelor’s preferred
-Previous experience with OUD, SUD, and/or MH populations
-Current Infection Control Certification
-1 years prior nursing experience and 1 year in a care management role
-MUST be able to work a flexible schedule
-Perform other duties as assigned
