Description

This position provides continuity of care, with the goal of permanent placement, for children through a family care management model that includes developing, expanding, accessing and linking resources in the community to the needs of the child throughout the child’s experience in the system, while documenting progress.

Specific job responsibilities and goals include but are not limited to:


• Functions as a contributing member of a family care team.

• Provides case management services, including assessment, planning, linking, monitoring, and advocacy, to assigned children and families from intake to discharge from the system.

• Ensures completion of Medicaid comprehensive assessment and implementation of appropriate case plan and treatment plan for all children and their families who are in the foster system of care. Ensure families are staffed with the multi-disciplinary team as needed or as is appropriate.

• Develops and ensures service/case plan includes: a realistic permanency goal that can be achieved within 12 months, measurable objectives related to the permanency goal, specific task assignments with timelines, and meets all legal requirements under Chapter 39.

• Reviews and updates service/case plans, every month or more frequent as needed, to ensure progress is being made towards permanency or modifications are being made in a timely manner.

• Completes monthly progress reports available to every stakeholder, including parents, and updates service/case plans whenever goals, interventions, or timelines need to be changed.

• Provides families education information in order to enhance their awareness of child abuse and neglect, promote family stability and a healthy family system.

• Maintains electronic and paper records of each case, ensuring that chronological notes and activities are accurate and entered timely into the appropriate data system.

• Ensures all legal/judicial work is prepared timely and in accordance with Florida Statute.

• Identifies and monitors children and families at risk, advises supervisor of their status.

• Refers children and families to appropriate community services and otherwise assures continuity of care for children and families.

• Treats all children and families with the utmost respect and common courtesy.

• Participates in staff development programs and opportunities. Participates in team meetings, client care and utilization reviews, and staff meetings.

• Establishes and maintains effective working relationships required for proper functioning within the role of case manager to include:

• Relationships as a member of the interdisciplinary team within the unit, particularly in the area of supporting other team members as appropriate and expected;

• Relationships with supervisory staff, emphasizing proper response to and use of supervision and appropriate reliance on supervisory staff to guide their activities during unusual circumstances;

• Relationships with community resource providers, particularly emphasizing facilitation of the referral process and providing continuity of care; and

• Relationships with children and families including awareness of and attention to developmental issues related to interpersonal functioning within the family environment.

• Completes other tasks or special projects as assigned by management.

• Other duties as assigned by immediate supervisor or leadership.

• Trained and authorized to access protected health information.


Requirements

Education & Experience: BSW/BA IN Human Services or MSW/MA IN Human Services; direct experience in FC/Adoption preferred


Physical Requirements: Ability to travel regionally on a regular basis and statewide as needed; Ability to bend, lift and carry 30 pounds.


Essential Functions:

• Ability to communicate effectively, verbally and in writing

• Ability to document timely in a clear, concise manner

• Must possess skills necessary to assess, interpret and deliver care-related information concerning the life-span development of children and families

• Must function as one member of a Family Care Team that manages a caseload children ranging in age from Birth to Seventeen

• Must possess a positive attitude concerning all aspects of working in a challenging environment, including significant patience and respect for children and families who can become quite angry and demanding, sensitivity to cultural needs and willingness to serve as a positive member of a working team

• Functions as a mandatory reporter, ensuring child safety and protection

• Must possess demonstrated knowledge of community resources

• Must possess a valid Florida Driver’s License and be insurable by the Agency’s current insurance carrier

• Ability to arrive/report to work on time and ready to work

• Ability to abide by principles of EEO compliance and a workplace of dignity and respect

• Ability to work cooperatively in a group/team setting

• Ability to show respect to others

• Ability to take guidance and direction from supervisors

• Ability to abide by core values and mission statement of the agency

• Proficiency in Microsoft Suites
