Job Description Job Description Position Summary: Responsible for assessing the risk and capacity of individuals referred by the community and addressing the needs of referred individuals. Principal Responsibilities: Conduct assessments to determine client risk and capacity. Establish an appropriate, comprehensive care plan for the client. Provide information designed to meet the needs of the client. Make appropriate and timely referrals and arrangements for services. Be responsible for the program’s follow up on all care plan items. Ensure that all New York State Office of Children and Family Services and New York City Human Resources Agency contractual obligations and agency policies are met. Advocate on behalf of the client to ensure that they receive everything to which they are entitled. Consult with and keep supervisor informed of needs and problems related to the client. Provide crisis intervention to stabilize client situations. Deliver cash, checks, food, mail and other items
