Job Description Job Description This is temporary for 4 weeks starting ASAP. Schedule is a little flexible according to program needs, 9AM-5PM or 10AM-6PM with a half hour unpaid lunch. The program is a permanent supportive housing program located in San Francisco. The program serves primarily low income and formally homeless seniors with complex and long-term social, mental health, substance abuse, and medical issues. The support services team is comprised of a full time program coordinator and two full time case managers and a full time Public Health Nurse. The team works together to ensure that all residents are provided with necessary resources to ensure housing stability. A primary focus of the program is to support and inspire housing community members’ independence and wellbeing through advocacy, education, and other supportive services. Job Description Job Description The Case Manager provides individual client case management support to clients of a residential substance abuse treatment program, as directed by the Clinical Supervisor and Executive Director (ED), ranging from intake and financial arrangements, through residential treatment and program completion Primary Responsibilities 1. Schedule and meet with clients in caseload, as required and as assigned. 2. Maintain current case management notes and other required documentation in client files. 3.
