Job Title: Care Manager (Social Worker) Duration: 3 months Contract to Hire, After FTE conversion salary - $60,000. Location: Hillsborough or Polk County, 75 - 90% remote. Shift Type: 8am-5pm EST, Mon-Fri Pay rate: $35 - $40 per hour Day-to-day responsibilities of this the role: Required to complete daily outreach calls based on member specific tiers Outreach calls are to check in on the members, follow up on their needs, remove any barriers to care for the members.
