Case Manager for Transitional Housing Program

Local non-profit housing organization is seeking a part-time Case Manager for our Transitional Housing Program. This individual is responsible for providing case management services, service coordination, and discharge planning for residents age 18-24 in our Transitional Housing Program. The Case Manager will maintain an independent caseload and ensure residents have access to appropriate services, supports, and resources necessary for their success.

Applicants must be able to demonstrate high proficiency in case management skills, have an understanding of human services systems, and heightened attention to detail. Must be highly organized, able to multi-task, set priorities, and have excellent written and oral communication skills. A high degree of proficiency with Microsoft Office software is required.

This individual is expected to exercise considerable initiative and independent judgment in achieving objectives while performing in accordance with established regulations, policies, and procedures.

Essential Duties and Responsibilities

• Treat all residents and staff with dignity and respect.
• Strive to deliver services in a safe and effective manner.
• Role model mature pro-social behavior.
• Maintain weekly contact with residents and document accordingly.
• Provide case management services to residents assigned by program staff.
• Conduct intake sessions, needs assessments, and individual service plans with each resident on theie caseload.
• Complete progress and case notes for all client-related activities within 24 hours of service delivery.
• Work with residents to develop life skills necessary to achieve self-sufficiency.
• Foster a supportive community among residents.
• Make referrals to support service agencies within the community as appropriate and support residents in navigating the process.
• Coordinate with agency and community resources to ensure residents have access to services, resources, and aftercare support needed to meet their personal goals.
• Arrange access to public assistance, social security, food stamps, and medical assistance appointments to ensure timely and continued receipt of benefits.
• Assist residents in scheduling and attending medical and dental appointments, mental health and substance abuse services, and obtaining medication refills, when appropriate.
• Assist residents in pursuing vocational and educational programs.
• Assist residents in developing a discharge/aftercare plan that includes services and supports needed to live successfully in the community.
• Collect program evaluation information from residents on caseload.
• Ensure confidentiality of all resident information.
• Follow all policies and procedures of the organization.
• Ensure compliance with all federal, state, local, and funding source guidelines.
• Participate in all staff development, training, and meetings as required.
• Other duties as assigned by Supervisor.

Qualifications

• Associate Degree in Social Service field (preferred) and/or a minimum of two years of case management experience in the Human Service field
• Proficient in Microsoft Office
• Must possess a valid driver’s license, active auto insurance, and vehicle
