Case Manager for Transitional Housing Program
Local non-profit housing organization is seeking a part-time Case Manager for our Transitional Housing Program. This individual is responsible for providing case management services, service coordination, and discharge planning for residents age 18-24 in our Transitional Housing Program. The Case Manager will maintain an independent caseload and ensure residents have access to appropriate services, supports, and resources necessary for their success.
Applicants must be able to demonstrate high proficiency in case management skills, have an understanding of human services systems, and heightened attention to detail. Must be highly organized, able to multi-task, set priorities, and have excellent written and oral communication skills. A high degree of proficiency with Microsoft Office software is required.
This individual is expected to exercise considerable initiative and independent judgment in achieving objectives while performing in accordance with established regulations, policies, and procedures.
