Job Description Job Description JOB DESCRIPTION HOME-BASED FAMILY EDUCATOR Under the supervision of the home-based services supervisor, the family development educator shall be responsible to: 1. Conduct weekly home visits with families lasting a minimum of one and a half (1.5) hours to a maximum of two (2) hours per session. Develop and implement weekly lesson plans in partnership with parents. Provide comprehensive services to families as outlined in the Head Start Program Performance Standards. Meet all program-required screening and ongoing assessment timelines with regard to health, education, disability and mental health. Collect, screen, and track all medical examinations and immunizations, working with families to obtain health information and follow-up as needed. Each month, in collaboration with parents, provide two developmentally appropriate socializations and/or fieldtrips for 0-5 year-olds and families on assigned caseload and complete classroom health and safety assessme

Job Description Job Description The Administrative Coordinator/Health Navigator will ensure that members receive health education materials and language assistance services as needed. This includes guiding members through the healthcare system, contacting members about health education services and programs, and assisting with scheduling health education classes and workshops. Monday-Friday 8:00am-5:00pm $18.00-$21.00/hour Responsibilities: Conducting member satisfaction surveys and maintaining a tracking log sheet. Checking member eligibility and documenting member contact in the medical management system. Mailing of health education materials to members and preparing health education, cultural and linguistic packets for members and providers. Assist members with scheduling medical appointments. Assist with scheduling health education classes. Assist members with transportation services and other related resources as needed. Contact providers for assistance with member specific cases.
